2. No Medical Advice

Quality of Life Home Care, LLC does not provide emergency assistance through its website, email, online forms, appointment-booking tools, or text messaging program.

In the event of a medical emergency, call 911 or go to the nearest emergency facility immediately.

Do not use a website form, email, or text message to report an emergency or request urgent medical assistance.

4. Eligibility

You must be at least 18 years old to submit personal information through our website or enter into an agreement with Quality of Life Home Care, LLC.

5. Requests for Home Care Services

Submitting a website form, scheduling a consultation, calling our office, or communicating with us does not automatically establish a caregiver-client, healthcare provider-patient, or contractual service relationship.

Before services begin, Quality of Life Home Care, LLC may require:

  • An initial consultation or assessment;

  • Verification of the prospective client’s needs;

  • Confirmation that the client is located within our service area;

  • Verification of caregiver availability;

  • Completion of required forms and documentation;

  • Insurance, waiver, Medicaid, private-pay, or other payment authorization;

  • Development or approval of a care plan; and

  • Execution of a separate written service agreement.

Any formal home care relationship will be governed by the applicable service agreement, care plan, authorization documents, and other written policies provided to the client or responsible party.

If a separate service agreement conflicts with these website Terms, the separate service agreement will control with respect to the home care services covered by that agreement.

6. Accuracy of Website Information

We make reasonable efforts to provide accurate and current information. However, we do not guarantee that all website information is complete, accurate, current, or free from errors.

Services, programs, eligibility requirements, caregiver availability, service areas, office hours, appointment availability, prices, payment requirements, insurance information, and other website content may change without notice.

Website content does not constitute a binding offer, promise, or guarantee unless expressly stated in a separate written agreement signed by an authorized representative of Quality of Life Home Care, LLC.

7. SMS and Text Messaging Program

7.1 Program Identity

Submitting a telephone number through a form does not, by itself, authorize us to send marketing text messages unless you separately and affirmatively provide the required consent.

Your consent applies only to Quality of Life Home Care, LLC and may not be sold, transferred, assigned, or used by another company for its own text messaging program.

7.5 Message Frequency

Message frequency varies depending on your interactions with us, the services you request, your scheduled appointments, your communication preferences, and the type of messaging program to which you consent.

You may receive recurring messages while you remain subscribed.

7.6 Message and Data Rates

Message and data rates may apply to messages sent to you from Quality of Life Home Care, LLC and to messages you send to us.

Your wireless carrier’s standard messaging, data, and other charges may apply. Contact your wireless carrier if you have questions about your mobile plan.

7.7 Opting Out

You may cancel the SMS service at any time by replying STOP to a text message from Quality of Life Home Care, LLC or by texting STOP to (267) 369-1356.

After you send STOP, we may send one final message confirming that you have been unsubscribed. After that confirmation, you will no longer receive messages from the applicable text messaging program unless you voluntarily opt in again.

Other commonly recognized opt-out requests may include:

  • CANCEL

  • END

  • QUIT

  • UNSUBSCRIBE

To rejoin the text messaging program after opting out, you must sign up again using an approved opt-in method or send an appropriate resubscription request, such as START, where supported.

Opting out of marketing messages will not prevent us from responding to a direct request that you initiate or from sending legally permitted service-related communications through another communication method.

7.9 Carrier Disclaimer

Wireless carriers are not liable for delayed or undelivered messages.

Delivery of text messages is subject to the availability and operation of your wireless carrier’s network. We do not guarantee that every text message will be delivered or received within a particular period.

7.10 Supported Carriers and Devices

The availability of text messaging may depend on your wireless carrier, mobile device, geographic location, mobile plan, and network availability.

You are responsible for using a device and mobile service capable of receiving text messages.

7.11 Changing or Transferring Your Number

You agree to notify Quality of Life Home Care, LLC if you change, deactivate, discontinue, or transfer the mobile phone number you provided.

You should opt out before transferring your mobile number to another person. You are responsible for ensuring that the telephone number associated with your consent remains accurate and belongs to you.

7.12 SMS Privacy

Personal information collected through our text messaging program will be handled according to our Privacy Policy.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors providing support services, such as customer service, technical support, hosting, or message delivery, may be permitted when necessary to provide those services.

Text messaging originator opt-in data, mobile numbers, and consent records will not be shared with third parties for their own marketing or promotional purposes.

8. Telephone and Email Communications

When you submit a phone number or email address, you authorize Quality of Life Home Care, LLC to contact you in response to your inquiry and according to the communication permissions you provide.

Depending on your request and consent, communications may be sent through:
